Leslie Jordan was on her way to the set of “Call Me Kat” when she died

Leslie Jordan was going to do what he loved, acting and making people laugh, in his final moments — he was on his way to the set when he died while driving … TMZ has learned.

Sources close to the late actor tell us Leslie was supposed to be filming Call Me Kat on Monday morning, but people working on the series became concerned when Leslie didn’t arrive more than an hour after he was called.

From there, our sources say it was a scramble to find Leslie — and the cast and crew eventually discovered he was participants in a car accident.

TMZ broke the story … Leslie was driving his BMW in Hollywood when investigators believe he suffered a medical emergency and crashed into the side of a building.

Sources close to the actor tell us he complained shortness of breath for the past 3 weeks and was so concerned about this problem that I actually made an appointment with a cardiologist.

Unfortunately, that was supposed to happen sometime next week.

​​​​​​While it is unclear whether this encounter would have revealed any serious health problems for Leslie, investigators’ current operating theory is that he suffered a heart attack while driving.

Directed by Leslie Call Me Kat. Maim Bialik posted a sweet tribute to the actor, writing: “They broke the mold when they made Leslie Jordan. He was a dear teacher and beloved friend. I will miss him so much – can’t imagine him being gone. Rest well my dear friend. “
